When you come in for your appointment, please remember to bring the following:
  • Your doctors referral form
  • Previous relevant Medical Imaging films / reports
  • Medicare card
  • Health Insurance information
Please remember to:
  • Arrive at the specified time to allow for registration and exam preparation.
  • Notify us upon arrival of any special needs or allergies
  • Wear comfortable clothing
  • Wear gym shoes if having stress testing
